INSTRUCTIONS

There are four sections to the survey:

I. Your stories and experiences across the board

II. Your perceptions and experiences relative to corporations/companies

III. Your perspective on corporate/company departments

IV. Your individual brand experiences

If you are working in a company/corporation, then all four sections will apply. If you are self-employed, perhaps only sections I and IV will apply. Or you can reflect upon prior positions to fill out Sections II and III as well.

You may participate in this survey anonymously or use it to promote your individual brand or company by providing name, title, etc.

You may also participate in this interview survey fully or in part. Naturally, the more fully you contribute, the more likely your stories will resonate with a cross-section of readers, and therefore, the more likely they are to be incorporated in the book.
